Reentry Coordinator

Job Summary Local nonprofit seeking a dedicated Reentry Coordinator to guide the successful reintegration of individuals transitioning back into the community after incarceration. In this vital role, you will connect clients with essential resources, support systems, and opportunities to foster stability and growth. Position located at County Jail, starting at part-time. Successful candidate will transition to full-time with benefits in less than six months. Duties Develop individualized reentry plans that include employment assistance, housing support, mental health services, and other resources Collaborate with County agencies, community organizations, and service providers to coordinate reentry plans Manage case files meticulously, documenting progress, challenges, and service plans Facilitate access to social work services and community programs tailored to each client's needs Requirements Experience in social work, case management, or related fields with a focus on justice-involved populations Strong skills in motivational interviewing Ability to write clear, comprehensive reports documenting client progress and service delivery Familiarity with social services resources available in the community is highly desirable
