Job Description
Bachelor's Degree or RN Required We are seeking a dedicated Service Coordinator to join our team. This is a part time position (maximum 20 hours per week). Hours will be split between travel within Livingston, Grundy and Caldwell counties. Must have valid driver's license and ability to pass background check. Hybrid schedule available following successful completion of probationary period. The Service Coordinator will be responsible for overseeing and coordinating services for individuals of all ages with Autism and Developmental Disabilities. This role involves meeting with individuals and families in their homes in Livingston County and surrounding areas.
Responsibilities:
- Meet with individuals and their family at their residence or within the community to provide service coordination (case management)
- Develop and implement individual service plans for clients with developmental disabilities
- Coordinate with behavioral health professionals to ensure comprehensive care
- Build and maintain positive relationships with clients, families, and community partners
