We are currently seeking a Team Case Manager to join our Ohio Home Care program. As a Team Case Manager , you'll work directly with individuals to assess their needs, develop personalized care plans, and connect them with essential services and supports. Key Responsibilities Conduct face-to-face eligibility evaluations and assessments (e.g., Adult Needs and Strengths Assessment) Must be available during business hours, 8:00 a.m. to 5:00 p.m., to complete all job duties, including but not limited to phone calls, home visits, and returning emails. Travel to and from the individual's home, not to exceed 2 hours each way. Assists the clinical team with special projects as assigned by the Clinical Supervisor. Functions as the team leader in the development, implementation and coordination of the individual's care plan, including the authorization of the amount, scope and duration of services, while meeting all qualifications and standards as specified by program guidelines; maintains ongoing contact with paid and unpaid services providers and/or caregivers. Develop and maintain individualized, person-centered care plans and Health and Safety Action Plans, make required reports to regulatory bodies, and complete incident reports as needed. Coordinate and authorize services across behavioral health, medical, and community-based systems Evaluates and periodically re-evaluates ongoing services to ensure the services meet the needs, goals, and objectives of the individual are appropriate and meets quality and cost effectiveness standards Assist individuals in accessing Medicaid and other essential services Monitor service delivery and ensure compliance with program guidelines Serve as the lead advocate and care coordinator for each participant Adhere to the CareStar Rule in performance of job responsibilities. Understand and comply with CareStar Policies and Procedures. Maintain confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense. Follow the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc. Minimum Qualifications Bachelor's degree in Social Work, Counseling, Psychology, Nursing, or a related field
Required:
Licensed Social Worker (LSW), Licensed Independent Social Worker (LISW), Licensed Practical Nurse (LPN) or Registered Nurse (RN) Minimum 1 year of experience in home and community-based services (within the last 5 years) Experience working with individuals with chronic conditions or severe and persistent mental illness Strong knowledge of Medicaid, behavioral health systems, and local community resources Why Join CareStar?
