Job Description
Position Overview The Case Worker provides individualized support and guidance to young adults transitioning out of foster care. This role focuses on helping young adults build stability, achieve personal goals, strengthen life skills, and maintain healthy support systems, including mentor relationships. The Case Worker serves as an advocate, accountability partner, and resource connector while helping participants navigate adulthood successfully. Flexible Schedule. Competitive Pay. Essential Job Functions Meet with young adults monthly, or more frequently as needed, to discuss goals, progress, barriers, and next steps Develop individualized goal plans focused on areas such as housing, education, employment, transportation, financial stability, and overall well-being Maintain consistent communication and provide ongoing encouragement and accountability Connect participants with community resources, programs, and supportive services Maintain accurate case notes, documentation, and participant records in organizational systems Assist young adults with completing applications, forms, and other necessary documentation Advocate for participants and help them navigate systems and services independently Collaborate with staff, volunteers, mentors, and community partners to support participant success Participate in team meetings, trainings, and organizational events as needed Non-Essential Job Functions Support special projects and organizational initiatives Other duties as assigned Requirements Prior Case Management Experience Strong communication and relationship-building skills Ability to work independently while managing multiple participants and priorities Passion for supporting young adults transitioning into adulthood Strong organizational and documentation skills Comfortable using computers and data management systems Valid driver's license and reliable transportation preferred Ability to maintain confidentiality and professional boundaries Preferred Qualities Trauma-informed approach to care and support Compassionate, dependable, and adaptable Ability to motivate and empower young adults Strong problem-solving and advocacy skills
