The Case Manager will be responsible for providing comprehensive care coordination, advocacy, and support services to elders and adults with disabilities with navigating Long-Term Services and Supports (LTSS). This role will involve direct client engagement, interdisciplinary team collaboration, and service planning. Key Responsibilities Developing, implementing, and coordinating person-centered plans that align with individual goals, preferences, and needs. Conduct assessments and develop individualized service plans. Coordinate with healthcare providers, community resources, and family members. Monitor client progress and adjust plans as needed. Conduct monthly participant outreach, which may include home visits to a caseload of approximately 50 participants. Services may be provided in a person's home or the community (for example, shared living or assisted living). Offer advocacy on the client's or family's behalf when needed services are not being adequately provided by an organization within the community's service deliver system Recognize potential conflicts of interest and refer consumers to next appropriate service to advocacy needs are met Maintain accurate and timely documentation. Participate in team meetings, supervision, and ongoing training. Qualifications Bachelor's degree in Social Work, Human Services, Psychology, or related field required. 1-2 years of case management, community health work, or related experience preferred. Strong organizational and communication skills. Ability to work independently and as part of a multidisciplinary team. Reliable transportation and willingness to travel throughout the state as needed.
