Adult Outpatient - Case Manager (Full Time) Community Care Alliance life insurance, sick time, tuition reimbursement, 403(b), mileage reimbursement United States, Rhode Island, Woonsocket 55 John A Cummings Way (Show on map) Apr 02, 2026 Adult Outpatient - Case Manager (Full Time) Community Care Alliance is seeking to add a motivated, task oriented, enthusiastic case manager to their Integrated Health Home Team! This position is interwoven into the Adult General Outpatient Department thus there will be no requirement to work evening or weekend hours. Our mission is to provide individuals with wraparound supports and services in order to improve and stabilize functioning so clients can achieve independence!
Duties and Responsibilities:
- Coordinating with a multidisciplinary team consisting of case managers, nurses, substance use specialists, clinicians and psychiatrists to enhance client care.
- Providing transportation to individuals, as necessary, such as to medical visits, court appearances, to pick up prescriptions etc.
- Providing assessment and referrals to various levels of care based on presenting concerns for mental health and/or substance use
- Working with clients to explore and access community resources, completing applications for benefit, and housing etc.
- Sessions occur within the agency, community and individual client homes
Job Incentives:

Education/Licensing Requirements:
- AA in a human services field required;
- BA/BSW in a human services related field preferred.
- Valid driver's license.
