Admin ($48.40 per hour) In person visit ($78.72 per hour)

Bachelor's degree in Human Services, Social Work, Human Sciences, or related field (or active RN license). Minimum 1 year of experience in social work, case management, or long-term disability services. 2 years of experience with DDSN or the Board of Disabilities. Valid driver's license and reliable transportation. Current PPD or TB test required.

Key Responsibilities:

Coordinate care plans and connect participants with appropriate service providers. Conduct quarterly face-to-face visits and annual re-evaluations. Maintain accurate documentation and ensure compliance with program policie s. Communicate with vendors, providers, and families to support participant needs.
