  • Full-time Case Manager Position Summary The Case Manager provides person-centered, trauma-informed support to individuals experiencing homelessness, helping them develop and achieve individualized service and housing plans.
This role requires strong advocacy skills, consistent documentation, and the ability to collaborate with internal teams and community partners. Evening and occasional weekend hours may be required. Essential Duties and ResponsibilitiesClient Support & Case Management Participate in and successfully complete initial HMIS training. Conduct assessments and support clients in developing goals to obtain and maintain permanent housing. Maintain knowledge of local community resources and ensure clients can access services both onsite and through community providers. Serve as an advocate on behalf of clients to ensure equitable access to needed services and supports. Build strong, collaborative relationships with clients to empower informed decision-making. Provide ongoing case monitoring, crisis intervention, and monthly problem-solving to address client needs. Documentation & Compliance Document all client interactions and services accurately and timely in the Homeless Management Information System (HMIS). If applicable, calculate rent and collect monthly payments in accordance with program guidelines. Ensure compliance with agency, funder, and governmental requirements. Check voicemail at least twice daily (Monday-Friday, excluding state holidays) and return calls regarding client care promptly: Calls received before noon: return by 5:00 p.m. the same day Calls received after noon: return by noon the following business day Program Operations Ensure beds remain at capacity by completing timely intakes and coordinating transitions to higher-level programs when clients meet eligibility criteria. Conduct weekly dorm meetings to review expectations, updates, and program rules. Perform daily, random dorm walkthroughs to help maintain a safe, clean, and supportive environment. Assist with agency programs, special projects, trainings, or certifications as requested. Attend all required meetings, including team meetings, staff meetings, trainings, and supervisory sessions. Maintain flexibility with scheduling, including occasional evenings or weekends, to meet client needs or complete required tasks. Resource Coordination Complete resource assessments for clients and make referrals to internal and external service providers as needed. Build and maintain positive working relationships with partnering agencies to enhance service delivery. Required Qualifications Bachelor's degree from an accredited university in Social Work or a related Human Services field. Experience providing direct case management services preferred. Proficiency with Microsoft Word, Excel, and PowerPoint; experience with HMIS is preferred. Strong interpersonal and communication skills, with the ability to maintain professional boundaries and ethics. Excellent written and verbal communication skills consistent with agency policy and HR requirements. Ability to work independently and collaboratively, demonstrating sound judgment and ethical decision-making. Strong problem-solving and critical thinking skills. Ability to establish rapport with clients and colleagues from diverse backgrounds. Physical ability to stand, sit, stoop for extended periods, and lift up to 15 lbs without assistance. Must pass all required trainings and certifications. Must possess and maintain a valid driver's license throughout employment.
