Job Description

Case Manager 250 Victory Ln, Conway, SC 29526 $16.01 an hour - Full-time $16.01 an hour - Full-time Make a difference. Advocate for choice. Build community. Ability Beyond Barriers is seeking a compassionate, detail‑oriented Case Manager to support individuals with intellectual and developmental disabilities, autism, and head or spinal cord injuries. This role is ideal for a human services professional who believes strongly in person‑centered planning , community inclusion , and advocacy . As a Case Manager, you will coordinate Home and Community‑Based Services (HCBS), help individuals access vital supports, and ensure services promote health, safety, autonomy, and meaningful participation in the community. What You'll Do Person‑Centered Planning & Service Coordination Lead and facilitate person‑centered planning with individuals and their chosen supports Develop, update, and monitor individualized Person‑Centered Service Plans in compliance with HCBS requirements Ensure informed choice, provider options, and conflict‑free case management principles Eligibility, Services & Authorizations Coordinate access to Medicaid waivers and state‑funded services Manage referrals, authorizations, service starts/ends, and required documentation Follow SCDHHS and BHDD‑OIDD case management standards Health, Safety & Advocacy Monitor health, safety, and overall well‑being Identify risks and coordinate mitigation, including critical incident reporting Advocate for individual rights, service quality, and community inclusion Community Networking & Monitoring Connect individuals to housing, employment, transportation, benefits, medical care, and community resources Monitor provider performance and service outcomes Participate in audits, reviews, and quality improvement activities Documentation & Billing Maintain timely, accurate case notes and records in electronic systems Complete billable activities in accordance with Medicaid Targeted Case Management requirements Emergency Response (Essential Employee) Support individuals during emergencies, severe weather, or declared disasters Conduct welfare checks, coordinate urgent services, and follow emergency procedures What We're Looking For Required Qualifications Bachelor's degree in social work or a related human services field or RN licensure Experience working with individuals with disabilities preferred Knowledge of person‑centered practices and case management principles Valid driver's license and ability to travel throughout the community Ability to pass required background checks Skills That Set You Apart Strong communication and documentation skills Ability to coordinate across multiple systems and agencies Advocacy mindset with respect for individual choice and dignity Organized, dependable, and able to manage multiple priorities Work Environment Community‑based position with regular home and provider visits Mix of field work and office documentation May require evenings, extended hours, or alternate locations during emergencies Why Ability Beyond Barriers?
