Job Description

Life Coach / Case Manager - Transitional Support Services Program (TSS) Positions available: Upstate Area of South Carolina South Carolina Youth Advocate Program (SCYAP) is seeking compassionate, motivated, and dedicated life coaches / case managers to support youth transitioning out of foster care into independent adulthood. This role is centered on building meaningful, trust-based relationships and empowering young people to develop the confidence, life skills, and resilience needed to thrive. A Life Coach / Case Manager will serve as a consistent source of guidance, encouragement, and advocacy during one of the most critical periods of a young person's life. This position will primarily work in the field. In this role, you will play a vital part in helping youth successfully transition to independence, supporting them in achieving stability in education, employment, housing, and overall well-being. Your guidance can help reduce barriers such as homelessness, unemployment, and lack of support systems, while inspiring long-term success and self-sufficiency. Key Responsibilities Ensure young adults have safe and stable housing, food, and other basic needs met Crisis management Build strong, supportive relationships with youth through consistent mentorship and engagement Provide individualized coaching focused on independent living skills, including budgeting, employmentreadiness, housing, and time management Support youth in setting, tracking, and achieving personal, educational, and career goals Offer emotional support, encouragement, and positive reinforcement to promote self-confidence andresilience Assist with job readiness, including resume development, interview preparation, and workplaceexpectations Connect youth to community resources such as education programs, healthcare, housing, andemployment services Advocate for the youth's needs and help them navigate systems such as education, mental health,employment, and social services Monitor progress and adapt coaching strategies to meet each individual's goals and challenges Maintain accurate documentation and participate in team meetings, trainings, and case coordination For consideration e-mail resume to: South Carolina Youth Advocate Program 140 Stoneridge Drive, Suite 350, Columbia, SC 29210 Please indicate position in subject line.
