Full-Time Schedule:
Monday to Friday 8am-5pm CST Position Summary We are seeking a compassionate and motivated PATH (Projects for Assistance in Transition from Homelessness) Case Manager to provide community-based outreach and support to individuals who are experiencing homelessness and severe mental illness. This role focuses on engagement, assessment, service coordination, crisis intervention, and advocacy to help individuals stabilize and maintain independence in the community. Key Responsibilities Identify and engage individuals experiencing homelessness and serious mental illness. Build rapport and establish trusting relationships with participants. Provide information and linkage to mental health, housing, medical, and community resources. Support participants with medication awareness, symptom management, and personal service plans. Conduct initial and ongoing assessments to identify needs in housing, finances, health, education, employment, and social support. Provide hands-on assistance in daily living skills within community settings. Advocate for participants across service systems and accompany them to appointments as needed. Participate in inpatient treatment planning and follow clients through hospitalization when required. Respond to and assist with crisis situations using team and supervisory support. Maintain accurate documentation including progress notes, assessments, mileage, and monthly reports. Qualifications Bachelor's degree in a human services-related field OR five (5) years of relevant experience in lieu of degree. Experience working with adults with severe and persistent mental illness. Knowledge of homelessness, community resources, and outreach services. Understanding of psychotropic medications and their effects. Strong communication skills including active listening, documentation, and collaboration. Ability to work independently in community settings and as part of a multidisciplinary team. Valid driver's license and reliable transportation (mileage reimbursed).
