In this vital role, you will empower individuals experiencing homelessness or housing instability by providing comprehensive support, connecting them to essential resources, and facilitating their journey toward stable, independent living. Your energetic approach and commitment to social impact will help transform lives and foster resilient communities. This position offers a rewarding opportunity to make a meaningful difference every day through proactive engagement, advocacy, and personalized care. Duties (each Case Management position may have slightly different duties) Conduct outreach to individuals experiencing homelessness on the streets or in shelters to build trust and assess their immediate needs. Develop individualized housing plans by collaborating with clients, landlords, service providers, and community partners. Advocate for clients during negotiations with landlords, ensuring fair housing practices are upheld and lease agreements are understood. Provide crisis intervention during urgent situations, employing crisis management techniques to stabilize clients and ensure safety. Maintain detailed case records; document progress, challenges, and service plans accurately. Educate clients on landlord-tenant law, Fair Housing regulations, addiction counseling resources, and available social services to promote self-sufficiency. Interact professionally in an inclusive environment. Qualifications Proven experience in Social Work, case management, or related fields with a focus on housing or homelessness services. Advocate effectively for clients with landlords and property managers. Ability to perform crisis intervention and crisis management with sensitivity and professionalism. Excellent communication skills with motivational interviewing techniques to motivate behavioral change. Certifications in addiction counseling or related areas are a plus; ongoing training in LIHTC compliance is desirable.
