Job Description
Case Manager - Substance Use Disorder Treatment (Abilene, TX) Serenity Foundation of Texas Serenity Foundation of Texas is seeking a compassionate and organized Case Manager to join our treatment team in Abilene, Texas . This position plays a vital role in supporting clients throughout their recovery journey by coordinating services, resources, and aftercare planning. Position Overview The Case Manager works closely with clinical and medical staff to ensure clients receive appropriate services, referrals, and aftercare planning while in treatment and upon discharge. Key Responsibilities Coordinate client services, referrals, and discharge planning Assist clients with housing, employment, legal, medical, and social service resources Maintain accurate and timely documentation in client records Collaborate with counselors, nurses, and treatment staff Advocate for client needs and promote successful transitions Ensure compliance with state and agency regulations Participate in team meetings and case reviews Provide ongoing support and follow-up as needed Qualifications High school diploma or GED required (Associate's or Bachelor's degree preferred in Human Services, Social Work, or related field) Experience in substance use disorder, behavioral health, or social services preferred Strong communication and organizational skills Ability to work with diverse populations Knowledge of community resources in the Abilene area is a plus Must pass background check and meet state licensing/training requirements Schedule Full-time Monday-Friday (some flexibility may be required) Compensation $16.00-$17.00 per hour, depending on experience (DOE) Benefits Include Health and dental insurance options Paid time off (PTO) plus 12 paid holidays annually Matching retirement plan Supportive team environment Opportunity to make a meaningful impact Why Join Serenity?
