Betty Hardwick Center is currently looking to hire a Jail In-Reach Coordinator/Case Manager in Abilene. : The Jail In Reach Coordinator/Case Manager is an integral part of coordinating and providing forensic services with the focus of restoration for persons who have been determined "Incompetent to Stand Trial" under the Texas Code of Criminal Procedure, Chapter 46B, and who are awaiting admission to a state hospital. The Jail In Reach Coordinator collaborates with local forensic teams to support active forensic waitlist monitoring of individuals pending criminal charges for court-ordered inpatient competency restoration services; provides communications with the courts regarding the individual's progress with mental health treatment, advocates for the individual's needs with the courts, and links to additional resources. This position will work closely with the jail navigators, community response teams, and jail in-reach educator and the post release case manager to coordinate care across all intercepts of the judicial and behavioral health systems. This position will also carry a small caseload comprised of individuals that are released from jail custody post or pre competency restoration. Caseload duties will be comprised of providing a variable level of support to consumers with mental health issues. This includes, but is not limited to providing assistance in accessing medical, social, educational, or other appropriate support services as well as linkage to more intensive services if needed, in addition to monitoring, assessment of service needs, service planning and coordination. This position is responsible for providing Rehab Case Management services within Levels of Care 1, 2 and 3 for adults within the Texas Resiliency and Recovery model of treatment. Responsible for insuring that the Recovery Plan is developed and designed based upon ANSA calculations and the Authorized Level of Care. This position requires a high degree of professionalism along with strong written and verbal communication skills. This position will have a schedule Monday - Friday, 8am - 5pm.
QUALIFICATIONS
Required:
A bachelor's degree from an accredited college or university with a major in social, behavioral or human services or is a registered nurse. (Social, behavioral, and human services are psychology; social work; medicine; nursing; rehabilitation; counseling; sociology; human development; physician's assistant; gerontology; education psychology; special education; early childhood intervention; or early childhood education.) •Must possess a valid Texas Drivers License and be an insurable driver. Driving is an essential function and employees must maintain an acceptable driving record/insurability while employed. •
Preferred:
At least one (1) year experience in providing mental health services to adults. Work experience with forensic populations. Knowledge of
DSM-IV/V
disorders. Jail In-Reach Case Managers earn $24.23 - $26.15 per hour (DOE); will be eligible for a one and two-year step increase, depending on satisfactory performance.
