Job Description
Performs minimally complex social service work. Responsible for assessment of client needs for emergent, urgent and routine services. Seeks authorization for services and provides oversight of services and evaluation of the effectiveness of services to stabilize mental health crises and promote recovery. Works under moderate supervision with latitude for the use of initiative and independent judgment. Provides skills/psychosocial rehabilitation to clients authorized for such services. This position requires shift work from Sunday- Saturday based on the clinic's schedule. Each case manager is required to participate on the MCOT on call schedule.
