Flexible Spending Account Essential Duties and Responsibilities:
Duties may vary based on specific program and position, and may include but are not limited to the following:
  • Under the general supervision of the Director of Forensic Services, the Jail Continuity of Care Liaison will work closely with individuals enrolled in the Jail Continuity of Care Program.
  • The role will provide mental health evaluations; facilitate actions put into re-entry plans; link those enrolled with community benefits application, and assist in community stabilization.
  • The role will require data collection, networking with community partners, and the building of professional relationships.
  • The role will allow latitude for some independent judgement, as it will oversee both the mental health treatment and forensic components of each individual's case. Role will advocate frequently within local criminal justice programs and court to ensure requirements are met.
  • Primary responsibilities include working with those identified for program eligibility, providing continuity of care planning, and building effective re-entry plans.
  • Work will primarily be conducted in outpatient settings, but may at times consist of jail-based work.
  • Work involves frequent in-person contact with individuals, families, and service providers. Competencies The competencies listed below are representative of the knowledge, skills, and abilities required to successfully perform the essential functions of this position:
  • Self-motivated with the ability to work independently and take initiative.
  • Strong organizational and time management skills.
  • Excellent verbal and written communication skills in the English language.
  • Ability to collaborate effectively with diverse individuals, interdisciplinary teams, and systems.
  • Proficient in accurately and objectively recording data and documentation.
  • Proficient in the use of computers using Microsoft Word and Excel.
Education and Experience:
  • Bachelor or master's degree from an accredited college or university with a major in Psychology, Social Services or related field required to be credentialed as a Qualified Mental Health Professional (QMHP).
  • A minimum of 6 months experience in behavioral health field is strongly desired as well as knowledge of mental illness, intellectual disabilities, medications, resources and the criminal justice system.
Certifications, Licenses, and Registrations:
  • Valid Driver's License required
