Job Description

Description:
We are looking for a compassionate and team-orientated Case Manager to join our Case Management team, providing needs assessment, crisis intervention, safety planning, information and referrals, and information about crime victims' rights and agency services to victims of domestic violence and sexual assault. The Case Manager assesses level of risk, creates and maintains service plans, and assists with agency intakes. The ideal candidate would have excellent organizational skills, strong communication skills and a drive to help others. Reports to :
Director of Case Management Supervises :
Assigned Interns and Volunteers Primary Functions Provide safety planning and crisis intervention for victims of domestic violence and sexual assault Conduct agency intakes, including needs and risk assessments and service planning Assess client needs and connect clients to resources, appointments, and agency programs Serve as primary point of contact for agency clients, assist in navigation and coordination of services, and monitor progress on service plan goals Apply professional judgment to client interactions, decision-making, and problem-solving in crisis situations Provide appropriate community resource referrals and maintain up-to-date information on community resources Provide information regarding protective orders, crime victim's compensation, Child Protective Services, criminal legal system, and all DCFOF agency services Provide Crisis Intervention for clients Facilitate peer support and/or psychoeducational classes for clients Maintain client files and records with confidentiality, with accurate and complete documentation Responsible for successful collaboration of both external and internal relationships for the delivery of sufficient case management services Utilize a strengths-based, trauma-informed approach with all clients Demonstrate knowledge, skills, and practices related to racial equity Participate in DCFOF staff and education meetings as requested. Requirements:
Prerequisites Bachelor's degree in social work, psychology, or related field Strong communication and organizational skills Demonstrated understanding of relationship violence and sexual assault, and trauma-informed interventions Preferences Excellent oral and written Spanish highly preferred Physical Requirements Ability to lift 25 lbs Ability to easily climb stairs Ability to sit for long periods of time
