The Case Manager will assess client needs, coordinate resources, and ensure access to appropriate services to improve the overall quality of life. The essential duties are Client Assessment; Resource Coordination; Advocacy; Documentation and Support. Bachelor's degree in human services, psychology, social work or related field with previous experience is desired.
