(GSI), a Fort Worth- based nonprofit organization, is seeking a compassionate and mission- driven Case Manager to join our team serving vulnerable adults in Tarrant County. Case Managers work directly with adults living in nursing home, assisted living facilities, and group homes to help ensure their safety, well-being, and access to appropriate care and services. This position offers a unique opportunity to make a meaningful difference in the lives of aging and at-risk adults through advocacy, coordination of services and ongoing client support. Key Responsibilities Conduct regular in-person visits with clients in residential care settings Monitor client care, safety and quality of life Coordinate medical, social, and community-based services Assist client in accessing benefits and resources Maintain documentation and communicate with interdisciplinary providers, facilities and families Advocate for vulnerable adults, and support person- centered care planning Qualifications Education Bachelor's degree in Social Work or related field Experience Minimum of 1 year case management, preferably with older adults and /or IDD populations. Skills and Knowledge Strong problem-solving and decision-making ability Ability to manage crisis situations with calm and professionalism Excellent written and verbal communication skills Knowledge of Dementia, IDD populations, Medicaid, and community services Computer literacy and documentation accuracy Additional Requirements Valid Texas driver's license, insurance and reliable transportation Ability to work evenings/weekends as needed and respond to after-hours calls (24/7) Must pass a criminal background check Must be willing to become a
Texas Certified Guardian Pay:
Up to $47,000.00 per year
