We specialize in Home and Community Services (HCS) and Texas Home Living Medicaid Waiver services. The role of the HCS Case Manager will include but not limited to the following:

Client Intake Needs Assessment Quarterly and Annual Service Planning Meetings Monitoring and Evaluation of Client Progression Scheduling of Client Appointments Documentation and Maintenance of Client Records Pay:

$15.00 - $20.00 per hour
