    • When you work through The Reserves Network, you are eligible to enroll in dental, vision and medical insurance as well as 401K, direct deposit and our referral bonus program Job Description The Service Coordinator supports residents and families by connecting them with resources, referrals, and case management services that promote employment, stability, and economic self-sufficiency.
    This role will conduct assessments, develop service plans, maintain documentation, coordinate with community partners, and provide follow-up support to ensure residents receive the services they need.
    Responsibilities:
    • Conduct resident interviews, needs assessments, and service plan development
    • Provide case management support, including referrals, follow-ups, appointment setting, and crisis assistance
    • Maintain accurate resident files, reports, case notes, referral forms, and related documentation
    • Build relationships with community agencies, service providers, resident councils, and local resources
    • Encourage resident participation in employment, training, and supportive service programs while tracking progress
    Qualifications:
    • Bachelor's degree in Sociology, Psychology, Social Work, or related field preferred
    • Five years of related case management experience with low-income individuals may substitute for degree
    • Strong interpersonal, written, and verbal communication skills
    • Ability to work independently, manage priorities, and maintain accurate documentation
    • Valid Texas driver's license, dependable transportation, and willingness to travel locally as needed
