Peer Specialist (Case Manager) - Transitional Housing 3178

Provide Peer support to people living with or in recovery from mental health and/or substance use conditions. Coordinates/facilitates services and activities of Milestone Transitional Housing. Facilitates discussion Groups to TTBH clients and provides rehabilitative services in both individual and group settings. Assist clients in developing personal goals for recovery and supports. Utilize tools such as the Wellness Recovery Action Plan (WRAP) to assist clients in creating their own individual wellness and recovery plans. Teach and role model the value of every individual's recovery experience. Models effective coping techniques and self-help strategies. Assist in obtaining services that suit that individual's recovery needs. Assist clients in developing empowerment skills and combating stigma through self-advocacy. Works under moderate supervision with minimal latitude for the use of initiative and independent judgment. At Center's discretion, position will include cross coverage and assignment to serve in other programs to provide treatment services.
