Job Description

Permanet Opportunity as a Case ManagerJob OverviewDrive meaningful outcomes for children and families by managing a dynamic caseload within the Permanency Program.

This high-impact, field-based role (up to 95% travel across San Antonio and surrounding areas) focuses on securing safe, stable, and lasting placements through strong coordination, advocacy, and hands-on support.

Key ResponsibilitiesManage permanency cases from removal through reunification or adoption, ensuring timely, stable placementsAssess child safety, family needs, and placement options to guide effective case planningPartner with caregivers, kinship providers, and adoptive families to create safe, nurturing environmentsCollaborate with courts, CPS, and public/private agencies to move cases forward efficientlyLead case conferences, home visits, and permanency planning efforts, including adoption readinessMaintain accurate, detailed case documentation and meet all reporting requirementsProvide crisis intervention and 24/7 on-call support on a rotating basisAdvocate for each child's long-term well-being, including family reunification and adoption outcomesRequirementsStrong knowledge of child development, family dynamics, and permanency planningAbility to travel and attend child and family visits as well as other work-related appointments and meetings after 5PM.– open to shifts, flexible – commitment to be available on call.

Comfortable working in the field, handling sensitive situations, and adapting to changing prioritiesValid driver's license and ability to travel extensively throughout the region
