QMHP CS I
perform assigned case management services for adult mental health ACT Team Consumers. Under the immediate supervision of a senior member of the AMH staff, but with allowance for some discretion in carrying out the details of the work.
EXAMPLES OF WORK PERFORMED
Provides rehabilitation skills training as deemed necessary in accordance with the Treatment Plan. Interviews or correspond with employment agencies, health and social welfare agencies, law enforcement officials, schools, and others to obtain significant background information which is needed for planning for the care of consumers. Works with psychiatrists, other physicians, and other medical disciplines to assist in the rehabilitation program of consumers and in the training, care, and treatment program of consumers. Develop job placements through contacts with community providers. Attends staff meetings and participates with other professional staff team members in the planning of the ongoing care and disposition of consumers. Prepare progress notes and other required documentation.
Experience:
Two (2) years of full-time paid case management experience in a mental health field, outpatient clinic, special school, or in a recognized case management agency, preferred.
Education:
Bachelor's degree from an accredited college or university in a human service-related field (e.g. social work, educational counseling, gerontology, psychology, or sociology.)
KNOWLEDGE, SKILLS, AND ABILITIES
General knowledge of mental illness and emotional components of physical illness; of rehabilitation methods; of basic casework principles and of general community welfare resources. Ability to work cooperatively and effectively with professional personnel; to exercise tact, initiative, and good judgement in dealing with consumers, relatives, and parents of consumers; to prepare reports and make recommendations for administrative use; to speak and write effectively; to relate to people and to interact with consumers. Ability to learn about and implement trauma-informed care practices within the scope of work for the position. Must have and maintain a level of general health that permits the performance of the job duties and ability to maintain regular attendance as set forth in Center policies and procedures. Employees must be able to perform unaided or with reasonable accommodation, the essential tasks, both in the training classroom as well as the job. Local travel 100%; transport clients. Ability to lift up to 15lbs.

Salary:
$45,000-$52,965 Annually; $21.6346-$25.4639 •Depending on experience and education•
