Senior Service's mission is to promote and protect the health and well-being of Davis County residents and their environment. The Case Manager provides guidance to enrich the lives of older adults and caregivers through advocacy, engagement, connection to resources and person-centered services for aging in place. Case Worker:
$ 24.16 - $ 30.75/hr Salary based on qualifications and experience. Must possess a valid Social Service Worker license from the State of Utah Division of Professional and Occupational Licensing or a Utah or National Certified Case Management Certification. Underfill as a Case Worker requirements: Graduation from an accredited college or university with a Bachelor's Degree in Social work, Gerontology, or a related field. Must become licensed as a Social Service Worker from the State of Utah Division of Professional and Occupational Licensing or become a Utah or National Certified Case Manager within two (2) years of hire date. Preference for employment experience working with older adults or older adult programs or services ie. Medicaid/Medicare benefits, hospice work, caregiving, etc. Preference for fluency in Spanish and English (fluency defined as proficiency level in both languages as evidenced by the ability to verbally interpret from English to Spanish and from Spanish to English, as well as the ability to translate written documents from English to Spanish and from Spanish to English). Day to Day Duties Schedules appointments with older adult clients and their caregivers to determine eligibility for services. Performs client visits in various settings and completes functional and social assessments on clients. Determines cost of services; assists with the development of written care plans for services for those eligible; authorizes service delivery; makes referrals to the community resources; arranges for the delivery of appropriate services to maintain clients independence. Completes regular follow-up client evaluations; recommends revisions to care plans as needed Acts as a liaison and advocate for clients. Maintains records on types of services being utilized; documents all client contacts; tracks service units on all clients/programs; compiles reports; prepares monthly billings.
