Job Description
The BridgeLine is accepting applications for a full-time Lead Case Manager position. This position provides community-based case management services to adults with brain injuries in Charlottesville and 13 surrounding counties. The incumbent will provide an assessment of service needs, coordination of services among providers, development of service plans, monitoring of needs, advocacy, consultation, and education for clients, families, and the community.
Qualifications:
Bachelor's degree in social work, counseling, human development, therapeutic recreation, or related field with at least years of experience in Case Management. Valid VA driver's license and a good DMV driving record. Strong verbal & written skills, basic computer skills required. Essential job functions of ourLead Case Manager:
maintains a case load of at least 10 active cases. reviews client goals and case notes for person-centeredness, progress and completion. provides guidance and mentorship to case managers on a scheduled and as needed basis. obtains information and make referrals for appropriate services. performs a comprehensive intake evaluation including the completion of all documentation as designated in the Case Management Program Manual and obtains all necessary records from outside sources. assesses assistive technology needs and seeks appropriate resources to meet needs. establishes a relationship with clients and their families / caregivers and communicates effectively with them and other support systems and professionals. supports clients / caregivers in developing their own goals and plans utilizing a person-centered approach. creates and facilitates support teams to include referrals, scheduling of meetings and delegation of duties. advocates on behalf of clients and their caregivers. participates in public awareness and community outreach activities, such as developing referral sources, attending health fairs, providing in-service trainings, attending fundraising events, coalition building and developing and/or facilitating support groups with a focus on rural counties; provides education and outreach to community partners, stakeholders and residents of rural counties within our catchment area.Supervisory responsibilities:
The Lead Case Manager distributes and monitors the flow of work for assigned group of members, interns, or volunteers. They provide training, assistance with resource identification and management, and escalates crisis and risk situations.What We'll Provide:
