Job Description

TriSource is seeking a compassionate and dedicated Case Manager to join the team of a Emergency Shelter in Ft. Belvoir VA , serving single adults experiencing homelessness. Clients in this program may face complex challenges including mental health diagnoses, substance use, physical or developmental disabilities, limited income, and chronic or long-term homelessness. The Case Manager plays a vital role in supporting clients as they work toward housing stability using best practices such as Housing First, Harm Reduction, and Trauma-Informed Care. This position is ideal for someone who is client-centered, resourceful, and passionate about helping individuals achieve long-term stability and independence. What You'll Do As a Case Manager, you will: Conduct comprehensive, strengths-based assessments to evaluate clients' physical health, mental health, social supports, education, life skills, and employment readiness Collaborate with clients to identify needs, set goals, and develop individualized service plans Assist clients in securing and coordinating services such as housing, employment, financial assistance, mental health care, and SSI/benefits Provide crisis intervention, coping skills support, and implementation of emergency procedures as needed Support clients with budgeting, money management education, and ensuring rent and utilities are paid on time Maintain accurate, timely documentation in client files and HMIS, including quarterly reviews and annual reassessments Perform other duties as assigned Required Qualifications Bachelor's degree in human services or a related field or equivalent professional experience At least 2 years of case management, direct services, or related experience Strong written and verbal communication skills Ability to manage competing priorities and work effectively in a fast-paced environment Valid driver's license and reliable transportation Ability to safely drive a 15-passenger van Willingness to work evenings and weekends as needed
Pay Rate:
$25-$27/HR
