Job Description

Responsibilities Cumberland Hospital for Children and Adolescents is a unique acute care, inpatient specialty hospital with Residential Treatment Center; staffed by the finest physicians and treatment teams. We treat patients, ages 2-22 who have chronic illnesses, brain injuries and neurobehavioral disorders with co-behavioral issues. Qualifications The Case Manager serves as provider of patient-centered services that promote coordinated, effective treatment by facilitating the communication exchange between patient/family, physician/treatment team and payor and managing timely transitions through the phases of rehabilitation. Communicate patient progress and continued needs with family/guardian/referrals source and payor source throughout treatment process Coordinates patient's transition outside the facility in a manner which assures continuity of care post discharge Coordinates patient centered treatment through the interdisciplinary team.
