Job Description

Job Description Catholic Charities of Eastern Virginia is seeking a qualified full-time Guardianship Case Manager to work out of their Norfolk office. The primary job of a Guardianship Case Manger is to coordinate and provide services for guardianship and conservator clients as required. The essential functions include maintaining accurate and timely client records; assist with providing documentation of income for Medicaid as needed; apply for federal, state and other applicable benefits on clients' behalf; maintain direct contact with clients including monthly client visits; budget and manage client fund; use independent judgement in making important decisions on clients' behalf; prepare legal reports for Department of Social Services; assist with legal reports for submission to Commissioner of Accounts; apply for applicable benefits for the client,. Guardianship related tasks are performed in accordance with the Code of Virginia. The ideal candidate will advise and educate clients on variety of financial topics and use professional discretion and judgement to help clients set up and implement an individually tailored Action Plan and Spending Plan that addresses the clients' housing goals, specific issues, and concerns. Requirements College degree preferred in human services, social work, nursing or related field Proficient in Microsoft Office Suite. Minimum of two years' work-related experience Two or more years' experience with older adults, individuals with chronic mental illness, or individuals with intellectual disabilities, including case management and care coordination Proficient in Microsoft Word, Excel, Outlook, and electronic record keeping Interpersonal skills Problem Solver Possess a working knowledge of finances and credit Understand default and loss mitigation solutions Acceptance of Pro-Life philosophy Must pass a background check and a drug screening. Duties Maintains accurate and timely records of all contacts with clients, professionals, and family members. Prepare and maintain all reports, progress notes, and other documents related to the client's case in the client's binders, and upload them on apricot system Assists with providing Medicaid and income benefit documentation to the accounting department Attend all department meetings, training sessions, and Guardianship Advisory Board meetings Maintains direct contact with clients and conducts monthly visits on a rotating schedule in home or day support settings. Performs guardianship, conservatorship, and representative payee duties in accordance with the Code of Virginia. Applies for federal, state, and other applicable benefits on behalf of the client Responsible for budgeting and managing client funds Reviews clients' monthly bills for accuracy and submits them for payment Opens and reviews daily mail, submits bills and invoices for payment, and responds to requests as appropriate. Must be able to operate a motor vehicle to transport clients to medical appointments, visits, and run errands on their behalf as needed. Uses independent judgment to make important decisions impacting the client's safety, benefits, and medical, legal, residential, housing, and care needs Manage emergencies while on call Maintains knowledge of community resources to support and refer clients served by the department. Performs other related duties as assigned, including lifting and moving objects weighing up to 25lbs. Responds to agency needs as appropriate, consistent with individual abilities, interests, and evolving community needs.
