Job Description

Position Details:
Under the direction of the Clinical Director, the Case Manager assists the Social Work department in providing resident, family, and community member services. Case Manager duties include implementing, monitoring and evaluating actions required to meet the residents' treatment needs. The Case Manager is responsible for transporting residents to required meetings or court, and attends court for foster care review or additional required court proceedings.
Responsibilities:
  • Complete case assignments, reports and paper work in a timely manner according to facility standards and established procedures
  • Establish rapport with DSS representatives, judges, lawyers, probation officers and family members
  • Schedule appointments to provide individual and family and short-term therapy
  • Assess the psychosocial needs of residents, caregivers, and families via psychosocial assessments; document assessment expectations for resident and parent/guardian treatment plans
  • Provide Psycho-Educational group therapy; completes related charting responsibilities
  • Establish interventions and outcomes, when necessary for residents within Hallmark programs
  • Complete A&D evaluations, and other behavioral health evaluations, when needed
  • Participate in the control of residents in crisis, as directed by Hallmark's Treatment Philosophy and Program Manual

High school or equivalent (Preferred)
Experience:
professional mental health and/or substance abuse field: 2 years (Required)
License/Certification:
Virginia Driver's License (in good standing)? (Required)
