JOB DESCRIPTION

Therapeutic Mentor
  • Position Part Time Job Responsibilities to include: Therapeutic Mentor provides Ability to offer youth structured, one-to-one strength
  • based support for the purpose of addressing daily living, social and communication needs and to learn new skills for improved functioning in the community.
  • Ability to support, coach, problem
  • solve and train guided by the goals that are established by youth's existing treatment plan. Provide in-home services to youth and their families including case management services.
  • Work with children and families in the family home and community Provide advocacy work with schools, courts, and other social service agencies. Attend FAPT meetings (Family Assessment Planning Team) for the referred youth enrolled
  • Must be able to use Comprehensive, home-based behavioral health assessment, with fidelity in regards to process notes, treatment plans and assessments and discharge plan Participates in case management meetings biweekly, responsible for progress notes and monthly reports, intake, assessments and ISP.
Overseer the evaluation of clients progress abilities and needs, Travel within communities served. Must be able to perform physical requirements: finger for computer operation and writing, frequently stand, sit and walk. Plan for professional growth. Other duties as assigned. Continued and efficient documentation of youth interaction and contact with providers regarding youth's progress; which is essential in this position Must have flexibility
  • be able to work after school hours and possibly Saturdays be available on a 24-hour basis when requested.
Mentors are required to use their own vehicle for transportation of youth and must be able to provide their own transportation to and from their assigned place or work Follows DWPS policies and procedures Support the DWPS's mission and vision. Must perform other duties as assigned Responsible for learning and carrying out the mission, policies and procedures of DWPS. Responsible for being a positive representative of DWPS in your appearance, actions and demeanor at all times.
Qualifications:
Bachelor' s degree in Social Work or related field. Two years' experience in community based child welfare service or an advanced degree in a social service field. Thorough understanding of issues related to parenting and child development. Knowledge of community resources. An understanding of family systems, the impact of abuse and neglect; substance abuse; mental illness; poverty; grief and loss, crisis intervention and trauma on children and their families. Must have valid Virginia Driver's License and valid automotive insurance at all times, must pass all background requirements, criminal, sex offender drug screening.
