Family Support Services, Inc is a licensed mental health agency, that is seeking to immediately employ individuals who are licensed in Virginia as a QMHP to provide services to individuals and families, that help to stabilize strengthen and support at risk children through community based counseling, collaboration and skill building. Clinicians provide 24-hour crisis support for our clients as well as adults who are in need of assistance with daily living skills. Assess the needs of clients to create a person centered treatment plan Aid in the development of specialized treatment plans and provide quality care coordination service to clients. Prepare notes/reports as required by payee, Medicaid and VA State License Regulations Be able to provide high quality services in individual's home or in community setting Provide person centered services based on the individual's needs including individual, family, group, and/or educational group services Develop individual service plans for each case serviced Collaborate with treatment and support teams this may include but not limited to therapists, social workers, case managers, and other professions Respond to crisis situations Complete daily progress notes, ISP's, discharge reports, service authorization reports, incident reports, and other case documentations according to agency procedures. Submit all documentations by agency deadlines. Identify and provide additional resources that support the needs of the client Coordinate and provides transportation to services and medical appointments as needed when appropriate Teach problem solving, and life skills Attend weekly supervision, required training and team meetings Must have clean driving record, valid vehicle insurance and reliable transportation Work Remotely No ( in person position)
