Competitive; based on experience Join a Team Making a Difference NOVA Community Services is seeking compassionate, motivated, and community-focused Qualified Mental Health Professionals (QMHPs) to provide Community Stabilization Services to children, adolescents, adults, and families throughout the Hampton Roads region. This position is ideal for professionals who enjoy working directly in the community and helping individuals navigate behavioral health crises, life stressors, and challenges that impact daily functioning. Services are provided primarily in clients' homes and community-based settings, requiring flexibility, independence, and strong clinical and documentation skills. Position Summary The QMHP provides short-term, intensive Community Stabilization Services to individuals experiencing a behavioral health crisis or significant emotional distress. Services focus on helping individuals and families develop coping skills, improve functioning, access community resources, reduce risk factors, and stabilize within their natural environment. Services are delivered face-to-face in homes, schools, shelters, community centers, hospitals, and other approved community settings throughout the service area. The ideal candidate is self-motivated, organized, comfortable working independently in the field, and passionate about helping individuals and families achieve stability during difficult times. Essential Responsibilities Direct Client Services Provide community-based stabilization services to children, adolescents, adults, and families. Deliver services primarily in client homes and approved community locations. Assist individuals experiencing emotional, behavioral, or situational crises. Teach coping skills, emotional regulation techniques, problem-solving strategies, and healthy communication skills. Support clients experiencing challenges related to anxiety, depression, grief, trauma, family conflict, school concerns, housing instability, financial stress, employment issues, and other life stressors. Assist clients and families with implementing crisis prevention and stabilization plans. Monitor client progress and adjust interventions as needed. Participate in treatment planning and discharge planning activities. Care Coordination & Resource Linkage Connect clients and families with community resources and support systems. Assist with referrals for housing resources, food assistance, employment services, healthcare services, educational supports, transportation resources, and other community-based programs. Collaborate with schools, healthcare providers, social services agencies, hospitals, and other community partners as appropriate. Advocate for clients and families when barriers to services exist. Documentation & Compliance Complete all service documentation accurately, thoroughly, and within required timeframes. Maintain detailed, clinically appropriate, and Medicaid-compliant progress notes. Complete service documentation within agency expectations and deadlines. Maintain accurate records of client contacts, interventions, outcomes, and service activities. Ensure compliance with DBHDS regulations, Medicaid requirements, HIPAA standards, and agency policies. Participate in audits, chart reviews, and quality assurance activities. Team Collaboration Participate in supervision, team meetings, case consultations, and training activities. Communicate effectively with supervisors and team members regarding client needs and concerns. Immediately report safety concerns, crises, or significant changes in client status. Maintain professional boundaries and ethical standards at all times. Required Qualifications Must be registered in Virginia as a QMHP-A, QMHP-C, QMHP-E, or QMHP-T Valid driver's license and reliable personal transportation required. Ability to travel throughout the Hampton Roads region. Ability to pass all required background checks and screenings. Strong verbal and written communication skills. Strong organizational and time-management skills. Ability to work independently and effectively manage a community-based caseload. Minimum one (1) year of Community Stabilization experience required Experience providing community-based behavioral health services. Experience working with individuals experiencing behavioral health crises. Knowledge of Virginia Medicaid and DBHDS requirements. Experience working with children, adolescents, adults, and families. Familiarity with local community resources and support systems.
