Job Description

ANEW Place is seeking a compassionate, organized team player and problem solver to provide case management services to adults experiencing homelessness in the Greater Burlington area and residing in or graduating from our congregate transitional shelter facility. ANEW Place is a faith-based homeless services agency equipping adults in the Greater Burlington area experiencing homelessness with the tools for lifelong change.
THE OPPORTUNITY
Reporting to the Program Manager, the Case Manager is responsible for providing clinical case management for guests at ANEW Place's sober shelter at 89 North Street. The Case Manager develops individualized goal worksheets, monitors progress towards housing, navigates housing resources, and aids guests with other needs. This position is also responsible for regular programming including life skills lessons, education on local resources and social-emotional wellness, and tenant skills.
Candidate Criteria:
Bachelor's degree preferred Specific Job Responsibilities Provide clinical case management related to housing goals which can include financial literacy, tenant skills, employment, benefits, applications, and community resources Conduct outreach to partner agencies on behalf of Anew Place and guests to improve outcomes, promote collaboration, and continuity of care Advocate on behalf of Anew Place and guests with housing agencies, private landlords, property managers, and service providers Complete clinical and administrative reports Develop and implement case consultations and clinical presentations to assist staff development Attend specialized training and participate in community meetings Assist with managing a waitlist, admissions, discharges, and ensuring house rules and policies are being followed. Support guest sobriety and recovery paths including conducting unsupervised UAs, and documenting clients' medications. Be alert and capable of handling building emergencies including fire/police/medical. Professionally and calmly handle conflict/crisis situations Act in a professional manner, including in verbal and written communication with guests, Board members, staff, volunteers, and community members. Be able to lift up to 30 lbs., climb stairs/step ladders, walk/sit/stand for extended periods of time.
