Job Summary:
The Case Manager is hired and supervised by the Director of Substitute Care. They are responsible for coordinating and facilitating ongoing support and services to children and youth experiencing severe emotional/behavioral disturbance and their families. They will engage in extensive collaboration with other agencies and support serving children and youth in need. In addition, they will be responsible for the supervision of assigned Support Staff working with clients.
Essential Duties & Responsibilities:
All direct-services positions necessitate training and preparedness in physical restraint techniques, given the at-risk population served by the employer. Provide ongoing support and services to children and youth experiencing severe emotional/behavioral disturbance and their families. Provide support and supervision to therapeutic foster care providers. Participate and facilitate ongoing treatment team meetings, participate in Case Plan Reviews, and participate in educational meetings. Develop and implement treatment plans which provide for the delivery of services and interventions as identified by the treatment team. Assist in developing natural supports for children and youth in within their communities. Work collaboratively with schools and other agencies providing services to children and youth experiencing severe emotional/behavioral disturbance and their families. Maintain client records to include writing weekly summaries, treatment team summaries and all other required paperwork relevant to providing client services. Provide crisis intervention and support to children and youth experiencing a severe emotional/behavioral disturbance and their families. Ability to cover, on a rotational basis, after hour crisis support (Level 2 & 3 staff only). Provide supervision, direction, and consultation to staff to ensure work is being performed according to established policies and practices. Regularly attends staff and other assigned meetings and training. Prepares agenda for team meetings to ensure an efficient and effective meeting. Train, mentor, and supervise assigned staff. Other duties as assigned, including financial, recordkeeping, and medication responsibilities.

Physical Requirements:
Given the at-risk population served, the position requires ongoing training and preparedness in the execution of physical restraint techniques. This may require running to catch a child. The "Handle with Care" training program articulates that individuals with back and neck limitations are likely to be hampered in the execution of the protocols and greatly increases risk of harm to self or student. Potentially long hours of sitting and driving in various types of weather and different times of the day to transport clients.
Qualifications and Education Requirements:
A minimum of a bachelor's degree in human services or related field is required. If not met, a waiver must be provided by DCF. Two years of human service delivery with children and families is preferred. Must be available for 2pm-9pm shifts and at least one weekend day shift. A valid driver's license, excellent driving record, and access to safe, reliable, insured vehicles are required. •
