Job Description

Are you passionate about treatment planning and clinical documentation? Northwestern Counseling & Support Services is seeking a dedicated individual to join our Adult Home and Community-Based programming as a "Pod Lead." This unique case management position will have a combined role that includes a small individual caseload, while holding a primary responsibility of administrative tasks associated with treatment planning for a small group of case managers. The Adult Case Management team serves adults with mental health and co-occurring disorders. We are seeking someone who is person-centered and passionate about working the population of individuals who intersect with criminal justice, legal, and substance use systems. This position is essential in supporting individuals to lead more independent and fulfilling lives.
Key Responsibilities:
Providing services across various community-based settings, focusing on supportive counseling, symptom management, daily living activities, social and interpersonal skills development, money management, and advocacy. Ability to build rapport with individuals who may be difficult to engage or resistant to treatment. Maintain a positive attitude, work well within a team, and stay organized to manage duties effectively. Collaboration and care coordination with community resources, agencies, and service providers. Develop and oversee treatment plans, service coordination, crisis intervention, and representative payee services. Being lead on treatment plan and associated documentation within a pod model.
Qualifications:
Reliable transportation with the capacity to transport clients in personal vehicle. Experience in mental health case management is a plus, but we are willing to train the right candidate.
