Job Description

The pay for this position is $44,850-$58,908.9150 annually. $500.00 sign on bonus Sometimes it takes only one person to make a difference in a life. As the Skills-Based Case Manager for adult services, you could be that person. The Skills-Based Case Manager works directly with individuals receiving services to build functional skills, address barriers to care and social and mental wellbeing, and support follow-through on treatment plans and referrals.
