Job Description

Recovery Navigator Program - Case Manager/Outreach Coordinator If you are applying for this position, you must go to the QBH website (www.qbhs.org) and complete the QBH application to be considered. Quality Behavioral Health is looking for individuals to join their Recovery Navigator Program in Asotin, Garfield, and Whitman County. The Recovery Navigator Program (RNP) provides intensive, field-based case management which helps participants access services that meet their needs when possible. These positions are public and highly visible, so experience with conflict resolution and de-escalation techniques must be a consideration. The intent of the Recovery Navigator Programs is to provide social services to individuals who intersect with police because of simple drug possession (ESB 5476§13) and/or people who have frequent criminal legal system contact because of unmet behavioral healthcare needs. The Program is intended to serve people who are at risk of arrest, or already have been involved in the criminal legal system. Referrals into the Program will come in on a 24-hour referral line from law enforcement, service providers, community members, and friends/family. Case Managers and outreach coordinators will respond to the referrals during their day time scheduled shift. Lived experience with Substance Use; the Legal system, or Mental Health is required in order to obtain Peer to Peer Certification which is required for this position. Experience providing services to individuals living with addiction from a harm reduction perspective is essential, along with case management, homelessness and co-occurring disorders preferred. Academic training in the social services field and in the area of substance use disorder treatment is desirable. RNP Outreach Coordinator job functions include, but are not limited to:
  • Outreach Coordinators will spend most of their time in the field visiting community-based organization and settings.
  • Act as contact point for all referrals to Case Management
  • Ensure effective and efficient communication and collaboration between all partners involved in referring and receiving referrals.
  • Provide screening outreach, and engagement to referred individuals who have yet to engage with their assigned Case Manager
  • Develop and maintain positive, collaborative relationships with all community partners and other service providers in order to bet serve participants
  • Manage and maintain referrals in database
  • Provide in the field outreach to engage referred individuals and help facilitate client engagement
  • Support Case Managers in finding individuals when necessary
  • Respond to community referrals and interact with individuals who might need case management or ongoing referrals to external services.
  • Provide short-term assistance while addressing the immediate needs of the individual.
  • Facilitate a warm hand off to the supportive services identified.
  • Follow-up with program participants in the community when there is indication of disengagement.
  • Prioritize responding to law enforcement calls in the beginning stages, with long term goal of being able to respond to any community-based and emergency response referral.
  • Coordinate with case management staff to meet the individual needs of new and existing program participants.
  • Collect and provide data points related to the individuals referred to the program and provide data to the Project Manager.
  • Demonstrate a plan is in place to ensure immediate access and response to individuals identified as needing services.
