Human Services Social Work Full Job Description Position Overview:
We seek a dedicated Housing Case Manager to work with individuals transitioning into or maintaining permanent supportive housing. This role focuses on offering tailored services that address barriers to stable housing, ensuring clients can sustain long-term tenancy with maximum independence.
Key Responsibilities:
Provide pre-tenancy support by conducting housing assessments, assisting with applications, and preparing clients to move into independent living. Develop and implement individualized housing support plans, identifying client goals, addressing challenges, and coordinating necessary services and resources. Assist with housing searches, eligibility determinations, and securing subsidies for clients. Offer ongoing tenancy support to help individuals maintain their housing by providing early intervention services and addressing issues like late rent payments or lease violations. Educate clients on their roles and responsibilities as tenants, ensuring they understand their rights and obligations. Mediate relationships between tenants and landlords, property managers, or neighbors and provide dispute resolution assistance. Facilitate connections with community resources to prevent evictions and support long-term housing stability. Assist clients in renewing housing subsidies and updating their housing support and crisis plans regularly. Collaborate with public agencies, local housing authorities, and private-sector partners to create and sustain housing resources for clients. Maintain accurate and timely documentation of all client interactions, service plans, and outcomes according to HHPO and DSHS standards. Qualifications Bachelor's degree in social work, human services, or a related field OR two years of experience in coordinating supportive housing or independent living services in a social service setting. At least one year of experience providing housing-related services to vulnerable populations, such as those with disabilities or behavioral health challenges. Strong understanding of housing laws, regulations, and compliance issues. Excellent communication skills with the ability to engage effectively with diverse populations. Demonstrated ability to handle crisis situations with empathy and professionalism. Strong negotiation skills and the ability to mediate conflicts effectively. Skills Strong understanding of Housing First principles , tenant rights , and harm reduction strategies . Ability to work collaboratively with public agencies, housing providers, and community partners to secure housing resources. Knowledge of housing subsidies, local housing systems, and services supporting individuals with complex needs. Excellent communication and mediation skills to foster relationships between tenants and landlords. Proficiency in case management and client documentation.
