SUMMARY DESCRIPTION
Coastal Community Action Program is looking to hire a Case Manager to work in the Ryan White & HOPWA programs. This position will assist clients in accordance with established guidelines, regulations, policies, and practices for three programs: Ryan White Case Management, Housing Opportunities for Persons With AIDS (HOPWA) and Ryan White Housing. Responsibilities include providing advocacy, assessments, case management, information, conducting outreach and referrals, client interviews, intakes, development and implementation of individualized case plans, incorporation of life skills training including budgeting, housing, employment, refer to legal experts and more, and enforcement of program policies. Assessment of eligibility for financial assistance for persons and families experiencing a recent loss or reduction in income or an increase in expenses and who would benefit from financial assistance. Monitor client's progress in accomplishing service plan goals. Coordinate distribution of funds to individuals and families requiring supportive services, emergency financial assistance, temporary assistance, transitional assistance, or tenant-based rental assistance. Work closely with the primary health care providers, case managers, state, and local agencies who provide supportive services for HIV positive individuals. This position demands a high level of cultural competency of the unique needs of the LGBTQIA++ community, communities of color, and other vulnerable populations. This position is a full-time position working from either our Olympia office, or our Centralia office, depending on location of the preferred candidate, serving clients in Grays Harbor, Pacific, Thurston, Mason, and Lewis Counties.
FUNCTIONAL RESPONSIBILITIES
Provide supportive services to clients: Client advocacy: Intervene with agencies/individuals to help clients receive appropriate benefits or services.
Consultation:
Consult with services providers utilizing their expertise for client benefit.
Networking:
Develop linkages between support systems to create an effective continuum of care.
Resource development:
Develop resources in the community to fill gaps in needed services for clients.
Family Support:
Strengthen and support care-givers in their role.
Crisis Intervention:
Provide short-term intervention within program guidelines. In coordination with the SES Manager, maintain confidentiality agreements and ensure strict confidentiality policy enforcement. Act as a resource within the community to promote and define resources available to persons with
HIV/AIDS.
Develop linkages and coordinate services with community resources Collects necessary documentation to verify program eligibility and continued compliance. Measures health and housing outcomes. Utilize PROVIDE or similar software to ensure appropriate information is collected and funding fee source is accurately determined. Provides outreach and housing-focused case management services. Assists client's living with HIV in locating and securing quality housing and/or increasing income. Triages clients who are requesting housing assistance services to identify and screen clients for appropriate services and/or funding. Works with the program supervisor/manager for final determination as to eligibility for rental assistance programs in accordance with applicable established program regulations and policies. Completes assessments, develops service plans, and connects clients to various community and government resources for support. Develops and updates, at a minimum four times per year or as needed, an individual housing plan addressing any barriers to housing and housing stability, income instability, and any current health care needs, including needed connections to insurance, medical case management and/or medication adherence. As well as identifying and goal setting for client's personal growth and well-being goals. Advocates on behalf of clients to decrease service gaps and remove barriers to services. Assists clients in developing and utilizing independent living skills and strategies. Provides information and referral to clients as requested or needed. Helps address mental health, housing, food insecurity, transportation, employment, and other barriers that may increase risk of homelessness. Maintains comprehensive knowledge of community services and resources. Follows up on clients housing stability. Maintains accurate, clear, legible, and timely progress notes & documentation, housing plan reviews and services provided in client records as specified in agency policy, program guidelines and performance standards. Processes payments for clients in housing and supportive services. To include HOPWA STRMU, PHP, TBRA and Ryan White Transitional Housing. Collects all required documentation for financial assistance (lease, utility bill(s), W9). Conducts housing inspections based on HUD requirements that correspond with funding sources. Prepares necessary program reports and records as requested by the supervisor and/or manager. Provides advocacy as needed for clients with property management; to ensure a successful housing placement as well as responding to property complaints, enquiries, and questions. Participate in case conferencing with case management team and/or community partners to evaluate and respond to client needs and plan for continuing services. Attends and participates in community meetings and outreach events to develop relationships with community agencies. Attends all required meetings. Facilitates groups and workshops on topics that will increase clients' knowledge of housing, basic financial and life skills, employment, or other topics as the need arises. Utilizes data and other relevant information to inform the creation or revision of housing programs. Assists other staff with assignments or special projects as necessary, and performs other related duties as required or assigned. Comply with policies & procedures and quality standards Represent the agency in a positive, compassionate & professional manner at all times. Support & promote positive teamwork across the agency. Treat all persons with dignity and respect. Ability to work as part of a multidisciplinary team. Ensure that confidentiality is maintained for clients and any paperwork, as required and sensitive information is handled appropriately. Must be able to appropriately intervene with agencies/individuals to help clients receive appropriate benefits or services. Maintain client electronic and paper files IAW governing ordinances Attend continuing education training as directed. Other duties as assigned
QUALIFICATIONS
AA or Bachelor's degree in social work, preferred. or an associate's degree in social work, or related field from an accredited program plus one (1) year of experience. Or An equivalent combination of education, certification, community service, training, and life experiences that demonstrate competency may also be considered. A minimum of one (1) year experience in case management is preferred. 1+ years' experience working with individuals experiencing homeless, or people living with disabling conditions, including severe and persistent mental illness/substance abuse and/or
HIV/AIDS.
Knowledge of community social service resources is a plus. Capable of working with people in a variety of situations and stress levels facing a wide range of barriers. Must be capable of maintaining strict confidentiality according to Federal rules and regulations. Must be able to maintain complete and accurate case records and statistics IAW Statewide Program Standards. Must possess strong organizational skills. Must have proficiency in Windows, Microsoft Office Programs and web based applications. Must possess the ability to communicate orally and in writing. Must have access to reliable transportation and maintain a valid Washington State driver's license and insurance. Must be able to work under regular and direct supervision from the Supervisor/Department Manager. Attentive to detail, consistently following written and oral instructions and guidelines, and maintaining a high degree of accurate and complete records. Ability to work compassionately with clients. Understanding of client advocacy and client self-determination. Experience working with individuals living with HIV/AIDS, substance use issues and/or with mental health needs, is a plus. Ability to use a holistic, trauma-informed, client-centered approach. Experience conducting outreach, a plus. Ability to work with diverse communities and demonstrate cultural sensitivity, a plus. Ability to effectively resolve conflict and cope with crisis situations. Must be self-motivated. Understand how to conduct services in an equitable manner. Ability to work under pressure, addressing multiple priorities.
