Case Manager

✔ Manage participant caseloads and individualized service plans ✔ Provide job training, workplace coaching, and skill development ✔ Coordinate services, resources, and participant supports ✔ Lead participant review meetings and track progress ✔ Maintain documentation and case records in Apricot ✔ Partner with families, employers, and community agencies

What You Bring:
