Job Description

The Stoughton Senior Center is seeking a compassionate, self-motivated Senior Center Case Manager to support adults 55+ in maintaining independence and enhancing their quality of life. A passion for serving seniors is at the heart of the role. This full-time position plays a vital part in advocating for older adults, assessing needs, coordinating services, and connecting clients with financial, medical, nutritional, transportation, and housing resources. Treats clients with dignity and respect in a fair and equitable manner. Interviews potential clients, families, neighbors and friends to determine need and type of service(s). Acts as an advocate for older adults. Plans housing options including in-home with supplemental services such as home health care, housekeeper, etc., assisted living and nursing home placements. Needs to be an inwardly motivated self-starter using creativity and insight in dealing with clients. Acts as an advocate for appropriate use of funding sources regarding needed resources. Assesses client need for home delivered meals and for cessation of program. Informs clients and the general public of available services. Completes various applications with clients (i.e. housing, financial, food assistance, etc.) and assists them in gathering eligibility determining information. Assesses safety in the home. Monitors services and discusses with client and clients' families. Assesses the need for change of services. Reports abuse and neglect to appropriate authorities. Refers clients to the appropriate person/agency to resolve housing and Medicare/Medical Assistance and insurance problems. Refers clients to legal advice when needed. Prepares correspondence, reports, and forms regarding outreach activities. Maintains knowledge of available services and resources for older adult population. Attends relevant in-service training, seminars, and workshops. Builds effective working relationships with clients, Senior Center participants, volunteers, community members and peers. Record keeping and record sharing with other agencies as is appropriate. Attends support group and community group meetings to explain outreach activities. Education, Training, Certifications & Experience A four-year degree in a human service-related field or comparable experience. Experience working with older adults. Knowledge of older adult programs and related resources. Certification in CPR or the ability to obtain within 6 months of hire Must be able to utilize a variety of advisory data and information such as financial statements, long-term care assessment information and forms, police reports, assistance and services application forms, referral information, hospital discharge information, elder law manual, Medical Assistance manuals, case management manuals, procedures, guidelines and non-routine correspondence. Ability to communicate orally and in writing with clients, clients' families/guardians, Senior Center personnel, medical care providers, nursing home personnel and social workers. Must be competent in Microsoft Office programs, other software applications, and the Internet. Conditions of Employment Must pass a background check, driver's license check, pre-employment physical, and drug screen. A valid Wisconsin driver's license is required at hire. A well-functioning automobile to be used extensively. Language Skills Ability to analyze data and information using established criteria, in order to determine consequences and to identify and select alternatives. Ability to compare, count, differentiate, measure and/or sort, as well as assemble, copy, record and transcribe data and information. Ability to classify, compute, tabulate, and categorize data. Must possess excellent interpersonal skills. Ability to advise and provide interpretation to others on how to apply policies, procedures, and standards to specific situations. Ability to maintain confidentiality at all times. Must have good organizational and time management skills. Must be a good listener. Must know the issues facing older adults. Mathematical Skills Ability to add and subtract, multiply and divide, and calculate percentages, fractions, and decimals. Judgment and Situational Reasoning Ability Ability to add and subtract, multiply and divide, and calculate percentages, fractions, and decimals. Physical and Mental Abilities Required to Perform Essential Position Functions Ability to operate a variety of office equipment, including computer, photocopier, telephone, etc. Ability to exert very moderate physical effort in sedentary to light work, typically involving some combination of stooping, kneeling, crouching and crawling, lifting, carrying, pushing and pulling. EnvironmentalAdaptability May be exposed to cigarette smoke on home visits. May be exposed to pets (sometimes can be unfriendly) May encounter unpleasant living conditions upon home visits. May be exposed to infected body fluids. May be exposed to potentially harmful cleaning agents. Ability to lift 25 pounds. Required to change from sitting, standing and walking positions repeatedly. Repetitive motion as required for computer keyboard use and handwriting. Need to be in and out of the car in temperature extremes.
