Wausau, WI Job Summary The Case Manager - GAPS Program provides client-centered, strengths-based case management services to refugee and asylee individuals and families enrolled in the GAPS program. Services are focused on short-term stabilization, system navigation, and connection to community resources, with an average service period of approximately 90 days. This role supports clients in addressing immediate needs, increasing stability, and building self-sufficiency through assessment, goal setting, referrals, and follow-up. While documentation and system use are essential, this position is not responsible for leading program-level data tracking or reporting. Key Responsibilities 1. Direct Case Management Services Provide short-term, goal-oriented case management to individuals and families enrolled in GAPS. Conduct intake and needs assessments to identify strengths, barriers, and priority goals. Develop and monitor individualized self-sufficiency plans in collaboration with clients. Provide follow-up support to ensure service connections and progress toward stability. Conduct home visits and community-based appointments as appropriate. Offer basic crisis support and referral to higher levels of care when needed. 2. Resource Navigation & Referrals Connect clients to community resources, including housing, employment services, public benefits, healthcare, schools, and other supports. Maintain working knowledge of local and regional service providers. Complete and track referrals to ensure timely access to services. Follow up with clients and partner agencies to address barriers and confirm service connection. Coordinate with internal programs to promote continuity of care and avoid duplication of services. 3. Transportation & Access Support Support clients in overcoming transportation barriers, including orientation to public transportation or coordinating rides when necessary. Assist clients in scheduling appointments and preparing required documentation. 4. Healthcare & Benefits Support Assist clients with scheduling medical appointments and understanding basic healthcare navigation. Support enrollment or follow-up related to eligible public benefits, as appropriate to the program. Provide education on accessing community systems independently. 5. Documentation & Compliance Maintain accurate and timely case notes, service plans, and referral documentation. Enter required information into agency databases and tracking systems. Ensure case files meet funder, state, and agency compliance standards. Support program reporting requirements as requested by the Program Manager. 6. Collaboration & Professional Practice Participate in team meetings, trainings, and continuous quality improvement efforts. Maintain trauma-informed, culturally responsive, and ethical service delivery practices. Collaborate with colleagues, volunteers, and community partners to support client success. Education, Experience, Knowledge, Skills & Abilities Bachelor's degree in social work, human services, nonprofit management, or related field preferred; equivalent experience may substitute. 1-3 years of case management or community-based service experience preferred. Understanding of trauma-informed and culturally responsive practices. Strong organizational and documentation skills. Effective written and verbal communication skills. Ability to manage a moderate caseload and prioritize tasks effectively. Proficiency in Microsoft Office and database systems. Additional Requirements Valid driver's license, auto insurance, and reliable transportation. Ability to transport clients when required. Background check and clean DMV record required. Ability to maintain confidentiality and professional boundaries. Occasional evening or weekend availability as needed. Environmental & Physical Demands Combination of office-based and community-based work. Moderate physical activity, including standing, walking, and occasional lifting (up to 50 lbs). Ability to manage stress and maintain professionalism in dynamic environments.
