Full-time Intensive Case Manager Definition of Work:

This is a professional community corrections position involving the implementation of intensive case management services to a population of high risk / high need clients in the criminal justice system.

Examples of Work Performed:

Conduct Initial Assessments for new clients, including LS/CMI Provide linkage and referral services to client population Provide intensive case management services to client population Update LS/CMI assessments for client population every six months Facilitate psychoeducational groups Conduct urine drug screens Managing urine specimens and oral swabs for the SRDRC Organize and conduct Treatment Plan Meetings and Treatment Plan Updates. Microsoft Office Required Education:
