HELP YOUTH BUILD A FUTURE THEY CAN BELIEVE IN
Case Manager - Independent & Transitional Living Program Full-Time Position Working with Youth & Young Adults Ages 14-24 A job can change your life. The right support can change theirs. Every year, young people in our community face homelessness, family conflict, instability, and uncertainty about their future. At YES House, we believe every young person deserves the opportunity to build a safe, stable, and independent life. As a Case Manager, you'll help youth take the next steps toward adulthood by connecting them to housing, employment, education, life skills, and community support. You won't just manage cases. You'll help build futures. Qualifications High school diploma or GED required; some college coursework in human services or related fields preferred. One to two years of experience working with at-risk youth, social services, or case management highly preferred. Strong organizational, communication, and interpersonal skills. Must be able to pass a background check and UA Demonstrated reliability, flexibility, and ability to maintain professional boundaries. Valid driver's license with reliable transportation for client-related travel.
WHAT YOU'LL DO
Help youth secure and maintain stable housing Support educational and career goals Connect clients to employment and workforce resources Develop individualized plans for success Provide transportation and referrals to community services Teach life skills including budgeting, nutrition, self-care, and independent living Build meaningful relationships that encourage growth, accountability, and confidence
