Job Description

NWTC Case Manager Position Description Mission Alignment:
This position supports the program's mission to provide a meaningful, sustainable, independent path leading to a life free from substance abuse, crime, incarceration, and dependency.
Position Summary:
The Case Manager for the Northern Wyoming Treatment Court Program is responsible for coordinating and monitoring participant progress through a court-supervised substance use treatment program. This role works collaboratively with the drug court team, including judges, attorneys, treatment providers, probation officers, law enforcement, and other service providers, to support participant recovery, ensure compliance with program requirements, and promote long-term stability.
Key Responsibilities:
Monitor participants' needs and track engagement within the program and coordinate referrals to any necessary outside services. Develop individualized case plans in collaboration with participants. Engage with participants on structured phase requirements. Monitor participant compliance with court requirements, including attendance, drug testing, and treatment participation. Maintain regular contact with participants through scheduled meetings and check-ins. Document all participant interactions, progress, and compliance in case management systems in a timely and accurate manner. Maintain documentation as required. Report and provide updates to the Program Director for court reports, treatment team meetings, and hearings. Participate in multidisciplinary team meetings and collaborate on case decisions. Ensure adherence to confidentiality standards and applicable laws (e.g., HIPAA, 42 CFR Part 2). May complete drug testing or medication counts for participants in line with program requirements if necessary. Support other program activities as required.
Qualifications:
Bachelor's degree in Social Work, Psychology, Criminal Justice, or a related field. Experience in case management, behavioral health, substance use treatment, or criminal justice settings preferred. Will train the right person. Knowledge of addiction, recovery principles, and trauma-informed care. Strong organizational, documentation, and time-management skills. Ability to work collaboratively within a multidisciplinary team. Effective communication and interpersonal skills, with the ability to engage with diverse populations. Familiarity with community resources and service coordination. Valid driver's license and reliable transportation.
Working Conditions:
Combination of office, court, and community-based work environments. May require travel within the service area. Evening or on-call responsibilities may be required in rare circumstances.

Reports to Program Director.
