Job Description

H2O Outreach and Education Specialist La Paz County & Gila County for dēhp Therapeutic Position Summary The H2O Outreach and Education Specialist is responsible for conducting community outreach, education, engagement, and resource coordination services throughout La Paz County and Gila County. This position focuses on connecting individuals and families to behavioral health, housing, supportive services, wellness resources, substance use treatment, and community-based programs. The Outreach and Education Specialist will work closely with community partners, healthcare providers, shelters, schools, probation/parole, tribal programs, law enforcement, hospitals, and underserved populations to improve access to care and community support services. This position requires regular travel throughout assigned counties and strong community engagement skills. Essential Duties and ResponsibilitiesCommunity Outreach & Engagement Conduct outreach activities throughout La Paz County and Gila County. Build and maintain partnerships with community organizations, healthcare providers, schools, shelters, and local agencies. Attend community events, health fairs, resource fairs, and outreach activities. Provide education regarding behavioral health, substance use, wellness, housing resources, and available community supports. Assist individuals in accessing treatment, transportation, housing, food resources, employment support, and healthcare services. Develop relationships with underserved and vulnerable populations. Education & Resource Coordination Educate community members on available H2O and community-based services. Distribute educational materials and resource information. Assist individuals with referrals using Unite Us, HMIS, or other approved systems. Support community awareness initiatives and prevention education. Provide information regarding crisis services, recovery resources, and supportive programs. Collaborate with community providers to improve continuity of care. Documentation & Reporting Complete outreach documentation and encounter notes in a timely manner. Maintain accurate records of outreach contacts, referrals, and follow-up activities. Track outreach outcomes and community engagement efforts. Submit required reports, logs, and documentation according to agency standards. Maintain confidentiality and comply with HIPAA regulations. Collaboration & Team Participation Participate in staff meetings, trainings, supervision, and community coalition meetings. Coordinate services with internal teams and external community providers. Support county-specific outreach initiatives and special projects. Maintain professionalism while representing the organization in the community. Qualifications High School Diploma or GED required. Associate's or Bachelor's degree in Behavioral Health, Social Work, Human Services, Public Health, or related field preferred. Experience in outreach, behavioral health, case management, housing services, social services, or community education preferred. Knowledge of community resources within Arizona preferred. Experience working with vulnerable populations, including individuals experiencing homelessness, substance use, mental health challenges, or justice involvement preferred. Strong communication and interpersonal skills. Ability to work independently and in community settings. Basic computer skills required. Experience with HMIS, Unite Us, or electronic documentation systems preferred. Requirements Valid Arizona Driver License. Reliable transportation and proof of insurance. Ability to travel throughout La Paz County and Gila County. Fingerprint Clearance Card preferred or ability to obtain one. CPR/First Aid certification preferred or ability to obtain. Ability to pass background screening if required. Physical Requirements Ability to sit, stand, walk, and drive for extended periods. Ability to lift up to 25 pounds occasionally. Ability to work in office, field, and community environments. Schedule Full-Time Travel & Transportation This position requires reliable personal transportation for travel throughout La Paz County and Gila County. Employees are responsible for their own vehicle expenses, including gas, maintenance, and mileage. Mileage and gas reimbursement are not provided for this position. Work Environment This position operates in community-based settings including shelters, clinics, outreach sites, schools, community centers, and other field-based locations throughout La Paz County and Gila County.
