The Community Health Outreach Specialist contacts patients, closing care gaps and helping remove the real-world barriers that keep people from getting the services they need. This is not a clinical or medical assisting role. Instead, the Community Health Outreach Specialist will primarily be speaking with patients on the phone, navigating EHR data, tracking outreach outcomes and coordinating with care teams. The selected candidate for this position must be bilingual in English and Spanish. You'll thrive in this role if you are: Culturally connected and empathetic, with an understanding of the challenges facing underserved communities. A people-first communicator who doesn't give up after one call and instead finds ways to connect with hard-to-reach patients. Data-minded and detail-oriented, comfortable pulling reports, documenting outreach and tracking outcomes. A resourceful problem-solver who thinks creatively when patients face barriers like transportation, scheduling or language access. Familiar with clinic, FQHC, or community health settings and how care coordination works. Common backgrounds that translate well to this role include community health worker, health educator, care coordinator, patient navigator, population health specialist, outreach coordinator, health plan case manager, social services coordinator, etc. Compensation and Benefits The compensation for this position is $23.00 - $24.00 per hour. This position is also eligible for the following benefits: Medical insurance (HMO and PPO options available) Dental insurance (HMO and PPO options available) Vision insurance 403(b) retirement plan 10 vacation days for first year employees 12 paid holidays 8 days of paid sick leave Life insurance Pet insurance Employee assistance program Essential Position Responsibilities Patient Outreach and Engagement Conduct outreach to patients who are due or overdue for preventive services, screenings, immunizations and follow-ups. Educate patients on the importance of preventive care and disease management. Participate in outreach activities, community engagement efforts and partnership-building initiatives. Care Coordination and Patient Support Identify and assist in closing care gaps by contacting patients to schedule appointments such as primary care visits, specialty referrals, lab tests and screenings. Assist patients with barriers such as transportation, language access and scheduling difficulties. Reporting, Data Tracking and Quality Improvement Document outreach activities in the EPIC EHR and outreach tracking tools. Track outreach outcomes and report progress toward quality goals. Provide feedback to leadership on outreach trends, barriers to care and overall outreach effectiveness. Participate in quality improvement initiatives and team meetings. Design and deliver training on trends, compliance requirements and regulatory updates. Act as the liaison between the organization and its health plans and payors. Education and Experience At least 1 year of patient outreach, care coordination or quality improvement experience in a clinic, FQHC, or population health setting. High school diploma/GED and/or equivalent experience required. Associate or bachelor's degree in relevant field preferred. Familiarity with electronic health records (EHR) systems. Skills Bilingual in English and Spanish required. Knowledge of healthcare quality measures, including UDS and value-based care metrics. Patient outreach and engagement strategies to close preventive and chronic care gaps. Proficiency in electronic health record (EHR) and population health management tools. Experience with data tracking, reporting and basic quality improvement analysis. Strong communication and motivational abilities for diverse patient populations. Cultural competency and experience working with underserved communities. Care coordination and collaboration with clinical and administrative teams. Ability to manage multiple outreach initiatives and meet performance targets. Strong communication and customer service skills. Ability to manage multiple tasks and meet outreach targets.
