Senior Housing Services Manager About Us St. Mary's Center is a community of hope, healing, and justice dedicated to helping older adults age with dignity by addressing housing instability, poverty, food insecurity, and social isolation. Through our Aging with Dignity model, we provide a continuum of person-centered services, including street outreach, coordinated entry, housing navigation, transitional housing, tenancy sustaining services, aging in place supports, and a vibrant Community Center, while empowering older adults to shape solutions through leadership and advocacy. Position Summary The Housing Outreach Coordinator leads housing-focused outreach activities targeting older adults experiencing unsheltered homelessness. The position coordinates outreach schedules, outreach events, weather-response activities, community partnerships, and the Senior Ambassador Program while supporting participant connection to housing resources and Coordinated Entry. Essential Duties and Responsibilities Outreach Planning & Coordination (30%) Develop weekly outreach schedules in coordination with SMC programming, Alameda County Health Care for the Homeless Outreach, and other service providers Coordinate outreach coverage throughout the service area Organize housing-focused outreach efforts Coordinate special outreach initiatives and community events Coordinate data collection and verification of information for preparation of services map and brochure for distribution Encampment Engagement & Housing Access (20%) Conduct outreach to unsheltered older adults including delivery of food, PPE and other items Support participant engagement and trust-building Facilitate referrals to Coordinated Entry and housing services Promote access to Housing Clinic and housing resources Emergency Housing & Weather Response Coordination (10%) Coordinate authorized hotel voucher placements, emergency shelter access Support timely placement activities for participants needing temporary or emergency housing, including during weather-related events Coordinate outreach, participant communication, and placement follow-up during emergency housing or extreme weather responses Track participant transitions from hotel placements or temporary shelter to appropriate housing pathways Senior Ambassador Program (10%) Coordinate Senior Ambassador activities including enrollment in educational activities and Community Health Worker training Support peer engagement opportunities Facilitate community participation and outreach support Community Partnerships (10%) Participate in regional outreach meetings Maintain relationships with outreach partners Coordinate outreach activities with community providers Coordinate volunteer activities to prepare hygiene kits and outreach materials Hotel Voucher & Emergency Response (10%) Coordinate hotel voucher placement when authorized Support emergency housing placement activities Monitor participant transition from hotel placements into housing pathways Coordinate participant communication and follow-up during temporary placements Documentation & Compliance (10%) Maintain outreach documentation Track outreach outcomes and participant engagement Participate in HMIS and Salesforce documentation requirements Support reporting and quality assurance activities Performance Expectations Outreach schedules maintained Consistent field engagement completed Housing referrals generated Community partnerships maintained Outreach documentation completed timely Ambassador Program activities implemented Minimum qualifications Three years' experience in social work, social services, or a related field We welcome candidates with a bachelor's degree in a related field or comparable experience working in homeless, residential, or social services. Demonstrated experience in outreach work, particularly with vulnerable populations or homeless communities, through work or lived experience Knowledge of resources and services available to homeless individuals, especially for seniors, in the Oakland area Ability to work independently and as part of a team, with excellent organizational and time management skills Dedication to collaboration, documentation, and meeting performance goals Bilingual Spanish/ English a plus Valid California Driver's License General Staff Expectations Promote a safe, respectful, and inclusive environment Support the mission and values of St. Mary's Center Maintain professionalism and confidentiality Follow agency policies and procedures Physical Requirements The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
