Outreach Manager / Program Director ________________________________________ Position Summary The Outreach Coordinator is responsible for identifying, engaging, and enrolling eligible members into clinical services through hospital, community, and field-based outreach. This role serves as the first point of human connection, ensuring members are approached with clarity, respect, and purpose. The Outreach Coordinator plays a critical role in bridging fragmented care systems by connecting members to services, supporting warm hand-offs, and ensuring accurate documentation of outreach activities. ________________________________________ Core Responsibilities 1. Member Outreach & Engagement Conduct daily outreach in hospitals, clinics, shelters, and community settings Identify and engage eligible members for services (ECM, mobile clinic, etc.) Clearly explain program benefits, services, and next steps Build trust with members through professional and empathetic communication Support initial intake and enrollment processes ________________________________________ 2. Hospital & Community Coordination Collaborate with hospital staff, discharge planners, and care teams Receive and act on referrals in real-time Maintain strong working relationships with partner organizations Represent HealMed02 professionally in all external settings ________________________________________ 3. Warm Hand-Off Execution Facilitate smooth transitions from outreach to care team (LCMs, providers) Ensure members are successfully connected to assigned care staff Track and follow up on engagement to prevent member loss Document all hand-offs and outcomes accurately ________________________________________ 4. Documentation & Data Entry Accurately document all outreach activities in EHR (Tebra or applicable system) Maintain real-time tracking of: o Attempts o Successful contacts o Enrollments o Barriers to engagement Ensure compliance with all documentation standards and timelines ________________________________________ 5. Compliance & Quality Adhere to HIPAA and confidentiality requirements Follow all SOPs and program guidelines Maintain ethical engagement practices—no coercion or misrepresentation Support audit readiness and data integrity ________________________________________ Required Qualifications High school diploma or equivalent (Bachelor's preferred) 1-3 years of experience in healthcare outreach, case management support, or community engagement Experience working with underserved or high-risk populations Strong communication and interpersonal skills Ability to work independently in field-based environments ________________________________________ Preferred Qualifications Experience in ECM, Medi-Cal, or managed care programs Familiarity with hospital workflows and discharge processes Bilingual (Spanish, Tagalog, or other community-relevant language preferred) Experience using Monday CRM ________________________________________

Pay:
$23.00 - $27.00 per hour
